Commuting Fatality NWR - LL # 11
Incident: Three employees (light vehicle drivers left Saham to travel over 1000km to Harweel in a private vehicle at the start of their shift. At 10 oclock at night after travelling over 700km at typical speeds of 150km/hr the driver lost concentration and drifted off toward the side of the road. He over reacted, over steering to the left and lost control. The vehicle rolled over and he lost his life as he was not wearing his seatbelt. The other two occupants were injured, one seriously.
Key Learning: Private commuting is our biggest killer – stay alive – use the bus.
Causes: Not wearing seatbelts leading to ejection from vehicle in a rollover caused by excessive speed and over reaction/correction by driver; Fatigue due to long journey causing vehicle to drift; Inexperienced driver at the wheel; vehicle not to the SP2000 specification allowing excessive speed, no IVMS monitoring and no roll cage protection; Employees chose not to catch the commuting bus from their home town due to long journey times.
