Incident Investigations
Tripod Beta Training Course arranged- book now
We are pleased to announce that the externally accredited Tripod Beta
investigation course will be held on the 5th to the 8th May 2013.
Tripod is the acknowledged investigation tool for PDO and contractors when a
serious high potential incident, fatality or Tier 1 Asset Integrity incident has
to be investigated and its use should form an integral part of the investigation
and influence the deeper management findings which other tools might not
discover.
All contractors who have a serious incident are required to head an
investigation and so should have a Tripodian or at least access to the services
of one. PDO should also have Tripodians in each Directorate to enable them to
conduct them in-house.
The course costs are RO100 for PDO staff and RO999 for contractor staff. Anyone
wishing to attend should contact Nigel Pitt, on npitt@kgonline.org to book their
place.
We can only provide the course if there are sufficient numbers of delegates and
it will also be on a first come first served basis with a blend of contractors
and PDO together.
We need to improve our incident investigation skills and this course is a key
step toward building capability to do that.
We will also be building a system to ensure Tripodians get the opportunity to
practice their newly acquired tripod skills by working together and on simulated
incidents so that once learnt the skills are not lost.
