Learning points:

• Ensure that temporary pipe-work are secured before applying pressure through them.
• Ensure proper line up prior bleed off operation and only choke valve to be used.
• Eliminate the use of flexible hose and swivel type elbows down stream of choke.
• Review the JSAs dealing with the pressuring of temporary pipe-work, and address the need for personnel to stay clear of the lines when under pressure.
