Recommendation and learned lessons

• Conducting refresh training for all supervisors about the importance of making an effective rescue plan to deal with any injury or fatalities cases, especially within critical jobs and conducting training for all employees to deal with any emergency case.
• Conduct a special training sessions for all workers about confined space hazards and safety precaution should be taken in similar cases.
• Cautious and warning signs should be used in any critical case like confined space.
• There must be an extreme direct supervision for all critical jobs.
• There must be a detailed handover between HSE department shifts.(log book can be used)
• Risk assessment should be done prior to start any job.
