EPE - Pedestrian knocked over by fork lift truck
Pedestrian knocked over by fork lift truck
After looking at all of our land sites, we found we were not managing site forklift/traffic/pedestrian hazards to ALARP or industry best practice. Our focus on hydrocarbon hazards may have contributed to a lack of attention to this risk. We encourage all EP land sites to review how they are managing this hazard, supported by the learning listed below: Some learning specific to this incident
Segregate pedestrian and vehicle areas
Road traffic risks for sites should be assessed, maintained and reviewed as changes occur.
Site driving policy should be reviewed with drivers and pedestrians to ensure they clearly understand their road safety risks
Some additional Best Practices for managing Road Traffic Hazards:
Optimal and enforced speed limits and traffic controls
Competent forklift drivers with ongoing refresher training
Optimal Traffic Management Plan that minimises traffic, reversing, etc., provides adequate space, segregates people and vehicles
Consider methods to improve forklift driver vision (e.g elevated cab, cameras, swivel chair, etc.)
Controls established when people are working in vehicle areas (e.g high visual clothes, limited vehicle access)
Reversing alerts (flashing lights, beeping horns)
