Lost balance and fell - Hip bone fracture
While racking BOPs, the driller tried to catch the winch line which was lowered through Rotary table to the cellar. In the process, he lost his balance and slipped in to the cellar between Wellhead flange and cellar pit, resting himself on the wellhead flange, causing fracture to his hip bone.
Effective supervision is essential to identify the hazards involved in their work as well as of others working with them. Application of `EMPOWERMENT¿ to stop work is the key.
