Rig floor safety awareness

After breaking the connection, while removing the breakout tong, the floorman was at the back of the tong and had placed both hands on tong lever assembly. The other floorman on the back-up tong unlatched the break-out tongs and pinched the injured floorman¿s small left finger between the jaw of the tong and the lever assembly.
Communication: Talk to each other. Hold useful toolbox talks. Team work essential part of safety. Always use ¿ What if.. ¿ scenario. Awareness: Place hands on handles color coded. Remember, You get only one set of hands
Failure to follow rules & procedures, Tongs have colored marks where to put the hands safely, Lack of safety awareness / lack of due care and attention
