FAC - Rig floor safety awareness V door
Floorman was standing at the top of the V door, in front of a single of DP, and had just picked up the tailing rope. When the winch operator picked up, the DP slid forward and struck the Floorman in the groin. He was treated as a FAC and returned to work.
Communication: Talk to each other. Hold useful toolbox talks. Due Care: Never lift a load without checking what might happen. Awareness: Stay clear of V-door when lifting is in progress
Winch Operator picked up the DP without due care and attention. Floorman was standing in front of the DP. Both men had been fasting for Ramadan and were probably hurrying the job so they could rest before the next connection.
