Handling Carrier Ramp Jack Supports
In preparation of rig move, the carrier was driven down from the ramp. The driller and two floormen attempted to drag the jack supports over the inclined ramp in order to place those onto the mud board base. While dragging the second jack support, it slipped onto the mudboard and landed on the ground. A floorman was working nearby the place where the jack support landed.
Use lifting gears to handle jack supports. Identify safer means of storage of the jack supports to avoid manual handling.
Failure to follow rules and procedures No lifting gear (crane/forklift) used to transfer the heavy (30 kg) jack supports. Inadequate planning / organisation Job could have been carried out more easily and safely with proper planning.
