Forklift with Elevated Load
Foreman was attempting to install a hexagonal kelly inside the scabbard by using the forklift. Roustabout helping, was standing too close to the elevated kelly. During the process, when the Foreman tried to remove the forks from underneath the kelly, the kelly landed on the right foot of the Roustabouts foot severely injuring him.
Keep a safe distance from elevated load of forklift or crane, ELEVATED LOADS ARE A HAZARD Proper communication can prevent an accident.
Failure to follow procedures / person standing close to elevated load on forklift. Failure in Communication / not communicating with each other about the consequences before attempting to remove the forks.
