PDO and Contractors suffered 2 fatal road accidents in Feb-2003.

A triple 3rd party fatality occurred on 19/Feb when a third party car overtook a truck that was turning left. The car hit the truck at high speed when overtaking in a zone where it was not allowed,

A high speed roll-over killed a drilling superintended and badly injured his 25 year old son. The main cause was speed and a tyre blow-out.

The purpose of the pack is to capture and disseminate the learning value of these incident to all levels in the organisation: PDO and Contractors. The pack is intended to assist YOU as a supervisor, site representative or foreman to explain and discuss the incident in a tool-box type meeting with YOUR staff. Emphasis should be on discussion in small teams (rather than a monologue by YOU as the supervisor).
