Fatality While Inflating Logging Truck Tire

On Jan 4, 2003 at 14:30 hrs, REW heavy duty driver/operator was fatally injured while inflating the right front tire of the Logging Truck. During inflation, the lock ring of the front right wheel flew off, hitting the operator on the face. He was rushed to the field Hospital where he was declared dead. The inflation and deflation of vehicle tires is a routine process when driving between Blacktop and Sand road conditions.

Recommendations:

  • Always check all the tire pressures before doing anything else.
  • Visually inspect the tire, rim, bolts and lock ring before inflation.
  • Use SLB recommended remote tire inflation equipment as shown in picture.
  • Inflation should be done when the tire is cold (driven less than 1 KM)
  • Position yourself away from tire and rim during inflation.
  • Recommended tire pressure for road conditions to be followed.
  • Use an approved safety retainer or cage and the correct tire handling tools.
  • If field repairs are required, use a portable safety retainer for the job.
  • Make sure the valve caps are in place when finished.
