2024 LTI#15 Roustabout fell from frac tank and suffered injury
On 21st Sep 2024 @ 9:30 AM, the operation was cleaning the mud tanks and transferring the water-based mud from rig tanks to Frac tanks. As part of the operation, the Roustabout climbed to the top of the Frac tank to monitor the mud level. While doing so, he leaned on the handrail of the Frac tank, which was not secured with the securing pins, causing the roustabout to fall from the Frac tank to the ground approximately 3.5m and land on his right side. Immediately, he was evacuated by rig ambulance to PDO Yibal clinic transferred to Ibri Hospital before being transferred to Shifa Hospital in Mabella, Xray revealed multiple fractures.
How do you ensure that all 3rd Party equipment is received with 3rd party checklist? How do you ensure the SOP of 3rd party-related operations is available? How do you ensure all out of sight area, equipment & activities are captured in your out of sight register? How do you ensure that all of the activities performed in your site are controlled with proper Control of Work?
The frac tank handrail was not secured with designated pin. The tank was mobilized to the site without 3rd party mobilization checklist. There was no supervision for the mud transferring job. No control of work was in place for the mud transfer job. No pre-job check was conducted on the frac tank before starting the transferring task
