Fractured foot LTI # 11
Incident: The job was to lower the flood light mast of portable tower light from upright to lay down in rest position. While lowering the mast approx 6-8 inches above the cradle, the wire rope got stuck between the edge of the sheave and the sheave plate. At that point, the Roustabout Foreman attempted to put back the wire rope under tension, onto the sheave. Suddenly the flood light mast moved down causing both his hands caught between the pulley and the wire rope.
Learnings: Effective risk assessment to be done identifying all hazards prior to the start of the job specifically when the job changes; Supervisor should Supervise and observe the job instead of doing the job by himself; Intervene, Stop and re-assess the hazard if any change or problem occurs during the job;Ensure guard/ stopper plate provided as per the maintenance checklist to prevent wire rope from jumping out off the sheave.
