• Inadequate compliance: Movement of vehicles on construction sites must be controlled by a banksman at all times. Drivers are not permitted to move vehicles without a banksman.
• Inadequate planning, management expectations and priorities: The first line supervisor was not given clear instructions on how to engage idle employees. The driver and the banksman were idle because of a general strike of equipment operators.
