Temporary Pipework fatalities have regrettably continued to blight our industry and we have regretfully experienced the fatal loss of two colleagues during the last three years, as a consequence of pressure induced catastrophic failure of miss-matched hammer unions. It is regrettable that we, as an industry, have failed in our obligations to send these colleagues home safely to their families. Furthermore, a number of high potential incidents and non-conformances have highlighted that we have been extremely fortunate on other occasions.
