WPH-7, While securing a material basket to trailer deck with chain & ratchet binder, the legs of the mat. basket broke through (punched through) the floor of the wooden deck trapping the IP toes of the right foot between the basket & trailer deck. This resulted in, avulsion of nail of right big toe and 2nd toe and four stitches
Check all baskets that arrive at your location. Are they fit for purpose and in good condition. (Pad-eyes fitted for securing chains?)
Is the basket marked with the SWL, if not don¿t use it
Are you aware of the weight that is put into the baskets, this should be marked on the accompanying paperwork
It is vital that these loads are secured to the flatbeds correctly. Items within the basket should also be secured Fatalities have occurred with shifting loads. If you don¿t know, have a look at SP2001 Load Safety and Restraining
Safety Meetings, Talk about body (foot) positions during loading, never put a part of your body under suspended or heavy loads
The type of basket used was not suitable for the job it was employed for. This type of basket (with legs) is for stacking in warehouses, not road transport. Little thought was given to the amount of weight loaded in the basket. There was no SWL sign on the basket and the people loading it did not have a clue what weight they were putting in. The method of strapping the basket down to the flatbed was wrong, cables/chains over the basket do not prevent loads shifting during heavy braking
