A rig electrician received fatal electrical shock 23rd May 2003 while repairing a microwave oven. The Rig Electrician ignored all three HIGH VOLTAGE warning signs on the equipment!

Immediate causes:
1) Procedures not documented 2) Failure to heed safety warnings 3) Competency

Underlying Causes:
1) Improper motivation; 2) Inadequate knowledge/skill 3) Inadequate procedure and work standard 4) Inadequate supervision/communication.
