BP Kamose Fatality: failed winch
A Briefing pack on a fatality which occurred on a BP offshore platform in Egypt as a result of lack of adequate risk assessment and changes in job procedures. A winch was being used together with 2 cranes to change out a crane motor. The winch hold down weld failed. A supervisor was dragged down and fell 4 m. He sustained fatal injuries.
