A High Potential Near Miss: Methanol Tanker
A truck and trailer loaded with an oil field type bulk tank containing 3000 Litres of METHANOL traveled from Muscat to make a "MOST URGENT" delivery to a production station. Methanol is a highly flammable and toxic chemical. The tank had no baffles. The "side-glass" was a length of plastic hose fastened by a wire to two nozzles. The vent had been sealed off with a piece of rubber and a clip. The drain valve that was to be used for offloading already had a length of 2" plastic hose connected. There was no facility to lock close the valve which could have very easily moved to the open position during transport. Hazchem markings were incorrect.
